Initial Appointment [Mashpee Office]

Service Description

$129 for a limited time! (or included in a Journey-To-Health Package) This is an initial 1-hour appointment with Dr. Forrest and Naomi at our office in Mashpee, MA. Receive a comprehensive holistic health assessment and an initial treatment plan to improve your health as effectively and naturally as possible. Treatment plan will include dietary recommendations, natural supplement recommendations, and other lifestyle recommendations to optimize your health. Lab tests, medical imaging, or referrals to other healthcare providers may also be suggested. We are happy to come to you for a house visit for an additional $30 fee (only available in Lehigh Valley, PA and Cape Cod, MA) If you are a Journey to Health program member: One week after this initial appointment we will meet again for a 40-minute follow-up to review your plan, answer questions, and check in regarding your health goals and concerns. Or choose to follow up as needed without being a program member. Weekly 20-minute follow-up appointments continue for as long as you are subscribed to our Month-To-Month Journey To Health program or until your 3-month program is complete. (If you purchase a Starter Package, which includes first 2 visits, you can continue your Journey To Health by purchasing appointments as-needed or by joining the Month-to-Month or 3-Month program)

Cancellation Policy

(1) Appointment Cancellation Policy: If you must cancel an appointment, please contact us at least 24 hours ahead of time to avoid charges. If you are unavailable at our second attempt to contact you for your appointment (5 minutes after your scheduled appointment time), your appointment will be considered cancelled without notice. Please see our policy section (https://www.archwayvitality.com/policies) for more information. (2) Retreat Cancellation Policy: If you need to cancel your retreat, you must do so before one month prior to the retreat start date (if you have paid in full before this date you will receive a 50% refund). Initial 20% deposits are non-refundable, but if you have paid in full and cancel by two months prior to the start date you will receive an 80% refund (total cost minus 20% deposit). Please understand that because of the limited number of rooms available and the fact that the retreat is coming up soon, we cannot offer any refunds once the retreat is one month (or less) away.
